Likely you know, what many times leads to the issue is the screw somehow tightening while the biner has flexed under some load.  Then, once the biner's load is largely removed, you're "screwed" unless ...

Often the trick is to apply the same weight or greater (e.g., having the belayer "take").  Again, you probably know or tried.

Yes, the loading issue you mention is obvious, but was not the case when it happened to me. I was at a bolted rap station on a ledge, so no loading. I never figured out why it happened. The only thing I can think of is dirt got into the screwing. I have since experienced "unhappy" (but not completely stuck) screw mechanisms when in the desert where sand/dust gets into the screwing. The best remedy for this is to turn the biner so the screwing faces down, knock on it and screw it back and forth to clean out the boulders that has fallen into it. Won't work once the screw gets stuck though, so you need preventative maintenance. Blowing into it might be tempting, but is probably more likely to "push" the dirt further down into the screw.

removing a nut after you fell on it and forgot your nut tool. (locking blade required)

This thread is great! Comical yet practical.  Well done and has been said always be the one carrying the knife.

Hello, my name is Inigo Montoya. You stole my route. Prepare to die. Honestly, cut webbing for a rap anchor (and like Benjamin says - remove old ones).

Holstered for cross draw on a climbing harness?  That seems very awkward.

How about a chalkbag/holster combo instead?

Hello.........The rack is on the rh. side.......the birds head gets tangled up in the cams. You have to be able to draw what you want when you want it....... ;)

I carry a knife when climbing for my certain and inevitable duel with a mountain lion.
