The 4th bolt is somewhat hidden from view if you're looking up from the bottom.

Location 

Starts just a few feet right of Quality Control 5.10c

This is a super fun route, I gave it 3 stars. It's a bit tiring near the top but this is a solid route with gym like holds, not unlike Industrial Bliss, but just a bit harder.
