Cob rock rock fall?

Yesterday we were climbing on Cob Rock and noticed that the flake that was on top of the boulder at the base of Empor had fallen off leaving fresh scars and obviously recent debris. I did not notice any posts regarding this...

Does anyone know what happened? Did it fall off by itself? Did someone pull it off (intentionally/unintentionally)? Was gear placed behind it? Did anyone get hurt?

Pretty sure it was precariously sitting up there and was intentionally trundled.
No injuries or accident.
