Pretty ridiculous that the rappel ring is backed up 3 times over, but no one bothered to check the entire loop of webbing or back it up...

Temple Crag rappel anchor- Why you should always check the entire rappel sling. This sling was otherwise new, but the pinch at the back was sharp and severed the webbing.
