Top roping safety

Has anyone seen or heard of a top rope double bolted chain anchor failing or rope being cut while top roping on a typical 1 pitch sport route. If so how do you mitigate this risk? I have never personally seen this happen, but I’m sure it has. I want to understand the conditions that can lead to these issue. Thanks in advance.
