I think a 9/64" t-handle hex wrench. That's from Gary Ballard's bolt removal docs if I remember correctly. He modifies it by putting in a vice and heating it with a blow torch and bending it to a 90 degree angle and then grinding it to length on a bench grinder. I made one of the 7/64" ones for 3/8" sleeves and it has worked well.

Can you tap items in (with a hammer and small punch or small screwdriver flathead)?
Then once loosened up remove with coat hanger wire turned into a v-threader tool basically..

Make a sleeve puller. Grind a notch in your shittist flathead screwdriver. It takes about 15 seconds with a cut off wheel and a grinder. A hacksaw would work too, but take longer. Make sure to leave enough room behind the hook to prevent it from breaking. It only needs to go into the hole about an inch so you could cut the blade short to allow for a better angle in the hole.

Dood, like this.

Guideline #1: Don't be a jerk.
