@ Benjamin Chapman. It is certainly possible that pilot error could have played a role given the fact that glassy quartzite offers very little margin for error when placing cams. I can only go by what I observed from the ground and what the leader told me. That is, that the cams were placed in their optimum expansion range and oriented in the direction of possible loading. Was an initially well placed cam bumped or partially dislodged as the leader struggled to make upward progress? Quite possibly, as both pieces that pulled were in close proximity to the leader's knees and feet just before he fell.
