a climber accross your chest with a heal hook on one nipple and a hand on the other or a nut on your thigh marking the extent of your sack with your age on one end. This could be updated through time to show how much gravity has effected your sack throughout life....

definitely do a full size rope coil backpack on your back with a few strands going over your shoulder and some tied off at your waist.

Bigger is better dude. If you go to a decent tattooer you won't have anything to regret. Get a topo of your favorite route covering your back, although the problem with back tattoos is you cant see them, and you have to be shirtless so other people can, and you know what they say about a climber who can't keep his shirt on.... Seriously though, some up close cam lobes or technical portrayal of a machined piece of gear is gonna be the most interesting over time.
